Where to Send Your Completed Agreements Questions



EUROPRACTICE member organisations wishing to gain access to the Cadence Online Support Knowledge Base will need to complete an Amendment to their Cadence End User Agreement (EUA). Two paper originals each with original signatures in ink must be sent to the Microelectronics Support Centre, Science & Technology Facilities Council, Harwell Science and Innovation Campus, Rutherford Appleton Laboratory, Chilton, Didcot, Oxfordshire   OX11 0QX, United Kingdom.

How to Complete

EUROPRACTICE Subscription Number: This is provided by the EUROPRACTICE Software Service upon applying for membership

End User: This is the full name and address of your institution. If your institution is commonly known by an abbreviation state this in addition to the full name

Effective Date: The date on which the Agreement is signed

Signature: Signature of legally responsible person. Must be an original signature, in ink

Name: Printed name (in capital letters)

Position: Position held by the signatory

Date: The date on which the Agreement is signed

 

If your Institution has an official stamp then please also include this below "date".



Where to Send Your Completed Agreements

Return BOTH original End User Agreements to:

What Next

RAL will contact the EUROPRACTICE Representative with information on how to access the Cadence Online Support Knowledge Base.

RAL will forward End User Agreements to the vendor for countersignature. Once countersigned by the appropriate vendor, RAL will return one original of each agreement to your EUROPRACTICE Representative.
